Location: Concord, CA 94520 (Hybrid)
Schedule: 5x8-Hour (09:00 - 17:00)
Contract Length: 13 weeks
Requirements:- High school diploma or equivalent combination of education and experience.
- Minimum of 1 year of recent experience in one or more of the following: Medical Scheduling, Patient Access. Healthcare, Administration, Call Center, Dispatch, Clerical Support
- Previous experience in Home Health or Centralized Healthcare Scheduling is strongly preferred.
- Experience coordinating high-volume schedules in a fast-paced environment with frequently changing priorities.
- Ability to schedule next-day patient visits based on clinician availability, productivity, and geographic drive time.
- Experience managing:
- Same-day visit requests
- Hospital discharges
- New patient referrals
- Ability to communicate effectively with patients, families, physicians' offices, clinicians, and internal departments.
- Proficient in Microsoft Office Suite, including Word, Excel, and Outlook.
- Knowledge of HIPAA regulations with the ability to protect Protected Health Information (PHI).
- Ability to work independently, prioritize multiple tasks, and remain organized in a fast-paced, frequently interrupted environment.
Summary / Duties:
Scheduler II will support Sutter Care at Home s centralized home health scheduling operations in Concord. The role ensures patients requiring next-day visits are scheduled appropriately while balancing clinician coverage areas, productivity expectations, and drive time. Daily responsibilities include maintaining service schedules, answering phones, coordinating with patients, physicians offices, clinicians, and internal departments, and updating required documentation. The scheduler will respond to changing priorities such as same-day visit requests, hospital discharges, new referrals, coverage gaps, and schedule adjustments. Success requires strong organization, rapid decision-making, attention to detail, and the ability to manage high-volume logistics in a frequently interrupted environment. This is a hybrid, non-patient-care scheduling function focused on efficient clinician routing, timely patient coverage, compliance, and PHI protection.
