Empath Home Health, a member of Empath Health is seeking a resourceful and dedicated Home Health Scheduler to support our home health team in Brandon, FL.

Schedule: M-F 8:30am-5pm
Location: Brandon, FL. Role is fully onsite.
What youll Do
- Enter scheduling data into Homecare Homebase HCHB and schedules clinical staff according to care plans/evaluations, location, and staff availability.
- Process all incoming patient referrals from physicians and acute/non-acute facilities.
- Verify Medicare benefit eligibility through on-line DDE checks; and if necessary, confirm beneficiary's status with other agencies by phone.
- Determine and communicate medical complexity/acuity of all referred patients and their orders to the Clinical Coordinator.
- Coordinate patient care with various members of the healthcare team, including physicians, physician's office staff, hospital discharge planners/case managers, and Territory Managers.
- Maintain confidentiality of patient information and all Empath Health operations.
- Support and carry out the organization's mission, vision, and values on a consistent basis.

What Youll Need
- At least one (1) year of experience in Home Health scheduling is strongly preferred.
- EMR experience with Homecare Homebase (HCHB) is ideal.
- High School Diploma or GED is required.
- Currently licensed as a Licensed Practical Nurse (LPN) in Florida is preferred.
- Demonstrates strengths in collaboration and partnership to attain strategic goals, organization, problem-solving, time management, decision-making, and follow-through.
- Advanced interpersonal skills including verbal and written communication skills.
- Able to work autonomously with minimum supervision.
