School-Based Physical Therapist - Aurora, Illinois

Key Qualifications:

• Active Illinois Physical Therapist license (required)
• Previous pediatric and school-based experience

Core Responsibilities:

• Deliver educationally relevant physical therapy services to special education students in an onsite school setting
• Address gross motor skill deficits by enhancing students' ability to access their educational environment
• Conduct assessments and develop individualized treatment plans that reflect each student's needs and goals
• Maintain accurate documentation, communicate progress, and participate in IEP meetings as required
• Support students' transition planning, including working with older adolescents (20-22 years old) on functional mobility and access
