Senior Case Manager

Position Overview

Sterling Case Management & Fiduciaries of Arizona is seeking an experienced, compassionate, and highly organized Senior Case Manager to join our Tucson team.

This position is responsible for managing complex fiduciary cases involving individuals with significant medical, behavioral, personal care, and social service needs. The Senior Case Manager works closely with fiduciaries, healthcare providers, families, attorneys, and community resources to coordinate services, advocate for clients, and ensure their safety, dignity, and quality of life.

In addition to managing an assigned caseload, this position provides leadership, mentorship, and guidance to other Case Managers while supporting consistent, ethical, and client-centered practices.

Key Responsibilities

• Manage complex and high-acuity client cases, including guardianships, conservatorships, and other fiduciary matters.
• Develop, implement, and monitor individualized care plans.
• Coordinate medical care, appointments, residential placements, and community services.
• Conduct client visits, assess care needs, and respond to changes in client circumstances.
• Collaborate with healthcare professionals, attorneys, families, and internal departments.
• Provide guidance, training, and mentorship to Case Management staff.
• Assist with resolving challenging client situations and coordinating appropriate interventions.
• Maintain accurate case documentation and ensure compliance with fiduciary standards, court requirements, and company policies.
• Participate in case management meetings, client care planning, and the assigned on-call rotation.

Qualifications

• Significant experience in case management, social services, healthcare, behavioral health, guardianship, fiduciary services, or a related field.
• Experience managing complex client situations and coordinating care for vulnerable adults.
• Strong leadership, communication, organizational, and problem-solving skills.
• Ability to work independently, manage competing priorities, and exercise sound professional judgment.
• Bachelor's degree in social work, psychology, human services, healthcare administration, or a related field preferred. Equivalent education and relevant professional experience will be considered.
• Proficiency with Microsoft Office and electronic case management systems preferred.
• Valid driver's license and reliable transportation required.

Schedule

Full-time, Monday through Friday. Participation in an assigned on-call rotation may be required.
