Job Description

We are seeking a Senior Social Worker to join our dynamic team. The ideal candidate will have strong experience in supervising social services in long-term care, case management, resident assessment, and regulatory compliance and a proven ability to lead teams, ensure timely and compliant documentation, and deliver safe, person-centered discharge planning.

Responsibilities:

• Plan, organize, and supervise the social services program for the Arizona State Veteran Home.
• Ensure timely completion of assessments and documentation per federal and state regulations and facility policies.
• Ensure compliance with PASRR processes and monitor Medicare notices to meet federal timelines.
• Supervise social services staff, including planning, assigning, reviewing work, and conducting monthly coaching and annual appraisals.
• Address staff issues and recommend disciplinary actions as needed; interview and select staff.
• Oversee clinically appropriate and safe discharge planning and communicate with stakeholders.
• Complete consults and referrals to Veterans Administration specialists.
• Collaborate with residents, families, and staff to uphold Resident's Rights.
• Assist with budgeting, operate within parameters, and monitor expenditures.
• Maintain a personal caseload and participate in meetings and committees.
• Participate in facility meetings, trainings, and Arizona Management System initiatives for continuous improvement.

Required Skills & Qualifications:

• Active social work license issued by the Arizona Board of Behavioral Health (LMSW).
• Master's degree in Social Work.
• Knowledge of CMS and state regulatory standards, PASRR, MDS, long-term care billing, and community resources.
• Skills in counseling, case management, resident assessment, care planning, analysis, problem solving, and supervision.
• Ability to maintain confidentiality, use facility IT systems, and build relationships at all organizational levels.
• Current negative TB skin test within the last 6 months or a year, and the ability to obtain and retain a valid AZ DPS Fingerprint Clearance Card.

Preferred Skills:

• One year of supervisory experience in a health care setting.
• Experience in long-term care.
