Service Coordinator

A social services organization on Long Island is seeking a Service Coordinator to join their team in Suffolk County. This role provides case management, housing stability support, and community-based coordination to help participants access housing, benefits, and related services.

About the Opportunity:

• Shift: Day shift
• Schedule: Monday to Friday
• Hours: Standard business hours
• Setting: Hybrid; office and field-based

Responsibilities:

• Provide short-term intensive case management for veteran households
• Develop and monitor housing stability plans tailored to participant needs and goals
• Conduct comprehensive assessments and maintain regular contact to track progress and service response
• Assist participants with housing search and placement, benefits enrollment, and access to community supports
• Complete documentation, reporting, and coordination with service teams to support compliance and service delivery

Qualifications:

• At least 3 years of experience in case management, homelessness prevention, rapid rehousing, or transitional supportive services

• BA in Human Services or a related field

• Experience in social services, or an equivalent combination of education and experience
• Demonstrated proficiency with Microsoft Windows, Word, Excel, and Outlook
• Strong communication and writing skills
• Ability to work in a hybrid model and in the field

Desired Skills:

• Experience working with homeless families or veteran services
• Familiarity with electronic database systems such as AWARDS
• Experience in intake, community outreach, or housing-related case management
• Strong organizational skills and the ability to multitask in a fast-paced environment
• Military cultural competency or prior service in the U.S. Armed Forces
