Service Coordinator

A home care agency is seeking a highly organized and compassionate Service Coordinator to oversee client services and caregiver
scheduling. This role ensures seamless coordination of care, regulatory compliance, and effective communication among clients, caregivers, and internal teams.

Responsibilities

  • Coordinate and manage client service schedules, ensuring full coverage and timely adjustments
  • Act as the primary point of contact for clients and caregivers regarding service needs
  • Onboard new clients, complete intake documentation, and develop care plans in collaboration with clinical staff
  • Monitor ongoing cases to ensure quality care and client satisfaction
  • Communicate regularly with aides to confirm attendance, address concerns, and provide support
  • Ensure compliance with agency policies and state regulations
  • Maintain accurate documentation and records in agency systems

Requirements

  • 1+ years of experience in a home care or healthcare coordination role
  • Strong scheduling and organizational skills
  • Excellent communication and interpersonal abilities
