Better Together Home Health is seeking a reliable and experienced RN Case Manager to join our skilled home health care team. In this role, you will provide clinical leadership, oversee patient care, and be a valued member for the interdisciplinary team. The ideal candidate has strong leadership and communication skills, skilled home health clinical experience, and a passion for delivering quality patient care.

PAY:  80k-85k per year

Qualifications:

• Active RN unencumbered license in Ohio or eligibility in accordance with applicable requirements.
• Minimum of 2 years of RN experience in post-acute care.
• Previous case management experience referred.
• Experience with wound care, tracheostomy care, ostomy care, catheters, eternal feeding systems, IV blood drawls and IV management preferred.
• Skilled Home Health experience and knowledge preferred.
• Strong communication, organization, problem-solving, and decision-making skills.
• Proficiency with electronic medical records and standard computer applications.
• Valid driver’s license and ability to travel within our service area.
• Must successfully pass required background checks and applicable screenings.

Responsibilities:

• Manage an assigned caseload and coordinate individualized plans of care.
• Complete comprehensive nursing assessments and ongoing evaluations.
• Collaborate with interdisciplinary teams.
• Educate and support caregivers and families throughout skilled admission.
• Complete accurate, timely clinical documentation in accordance with Medicare requirements and agency guidelines.
• Participate in IDT meetings and communicate changes in patient conditions promptly.
• Provide compassionate skilled care in patients’ homes and other residential settings
• Address patient, family, and employee concerns and help resolve issues.
• Supervise and direct STNAs.
• Complete new home health patient admission assessments and documentation.
