Hiring Range: $49.732.80-$64,667.20
The actual compensation for this position will be determined based on experience and other factors permitted by law.
Responsibilities:
• Facilitates partnerships with intra-hospital departments and essential community networks to ensure successful development of social services programs to meet patient needs. Connects patients to visiting nurse agencies to address medical needs following hospital discharge. Connects patients to subacute rehab when indicated for continued rehabilitative services and/or acute rehab when appropriate.
• Oversees primary needs of patient, including assessment of patient and family/caregivers and develops a comprehensive discharge plan
• Provides consultation to complex patient and family needs within the area of expertise and in collaboration with the interdisciplinary team
• Monitors patient care/plan of care through assessment, evaluations, and or patient medical record. Advocates for resources and identifies potential barriers to moving the patient through the care continuum
• Maintains ongoing dialogue with patient/family/caregivers/interdisciplinary team members both within the hospital setting and community setting to ensure a safe and appropriate transition of care plan/discharge plan.

Minimum Qualifications:
Education, Training & Experience:
• Bachelors degree in Human Services or related field required.
Work Experience:
• Recent experience in acute care setting preferred
• Knowledge of insurances and prior authorization process preferred
• Knowledge of community based organizations (ie: Nursing homes, CHHA's, Assisted Living/Adult homes) preferred
