ALIYA on 87th is seeking a compassionate and organized Social Services Assistant to support the Social Services department in meeting the psychosocial needs of our residents. The ideal candidate will have strong communication and interpersonal skills, a genuine commitment to resident well-being, and the ability to work collaboratively with residents, families, and facility staff.
Key Responsibilities
- Assist with the planning, coordination, and implementation of the facility's social services programs.
- Support residents and families with adjustment to the facility, including admissions, transfers, and discharges.
- Assist residents with identifying and addressing social, emotional, and psychological needs.
- Maintain accurate and timely documentation, including resident assessments, care plans, and progress notes.
- Participate in care plan meetings and collaborate with the interdisciplinary team to support resident needs.
- Serve as a resource for residents and families, addressing concerns and helping resolve issues.
- Assist with discharge planning and coordinate with community resources and outside agencies.
- Advocate for residents' rights, dignity, and personal preferences.
- Assist with identifying residents who may require additional social services support.
- Ensure social services documentation and practices comply with applicable federal and state regulations.
Qualifications
- Bachelor's degree in Social Work or a related field preferred.
- Previous experience in social services, preferably in a skilled nursing facility or healthcare setting.
- Knowledge of long-term care regulations and resident rights preferred.
- Strong communication, organization, and interpersonal skills.
- Ability to work effectively with residents, families, and interdisciplinary team members.
