A skilled nursing and long-term care center in Chicago’s Uptown neighborhood is seeking a compassionate, organized, and resident-focused Social Services Assistant / PRSC to join our team. This role supports the social services department and helps ensure residents and families receive the guidance, resources, and support they need throughout their stay.

The ideal candidate is a strong communicator who is comfortable working with residents, families, interdisciplinary team members, and community agencies in a fast-paced nursing home environment.

Responsibilities

  • Assist the Social Services Director with resident assessments, care planning, discharge planning, and documentation
  • Support new resident admissions and orientation processes
  • Meet with residents and families to identify needs, concerns, and available resources
  • Help coordinate discharge plans, community referrals, transportation, and follow-up services
  • Maintain accurate and timely resident records and social service documentation
  • Participate in interdisciplinary care plan meetings
  • Assist residents with Medicaid, insurance, and other benefit-related paperwork as needed
  • Help address resident grievances and concerns in a professional and compassionate manner
  • Coordinate resident appointments, outings, and communication with outside providers and agencies
  • Support psychosocial programs and resident activities that enhance quality of life
  • Ensure compliance with all state, federal, and facility regulations related to social services

Qualifications

  • Previous experience in a skilled nursing facility, long-term care, rehabilitation center, or healthcare setting preferred
  • Experience as a PRSC, Social Services Assistant, Admissions Coordinator, or related role preferred
  • Knowledge of discharge planning, resident rights, Medicaid, and long-term care regulations is a plus
  • Strong organizational, communication, and interpersonal skills
  • Ability to maintain confidentiality and handle sensitive information appropriately
  • Proficient in Microsoft Office and electronic medical records systems
  • Ability to work collaboratively with residents, families, and interdisciplinary teams
