White County Rehabilitation and Nursing is seeking a Social Services Coordinator to support residents and families and assist with care coordination, discharge planning, and admissions.Pay: $18.00-$20.00 per hourCNA: $19.50-$23.22 per hourScheduleFull-TimeMonday-FridayResponsibilitiesProvide support and assistance to residents and their familiesAddress resident and family concerns and help coordinate appropriate servicesParticipate in care planning and interdisciplinary team meetingsAssist with discharge planning and coordination of outside servicesMaintain accurate and timely documentationAssist with admissions and intake processesCoordinate with hospitals, referral sources, case managers, and internal departmentsSupport a smooth transition for residents entering and leaving the facilityQualificationsExperience in social services, skilled nursing, long-term care, or healthcare preferredExperience with admissions or discharge planning preferredCNA certification preferredBachelor's degree in Social Work or a related field preferredStrong communication, organizational, and interpersonal skillsAbility to work effectively with residents, families, and an interdisciplinary teamBenefitsMedical, dental, and vision insurance401(k)Paid time offSupportive team environment
