Key Responsibilities:
- Lead and manage Social Work Supervisor, Social Workers, Behavioral Health Specialists, and Social Work Assistants.
- Handle hiring, training, supervising, and coaching of staff.
- Oversee quality of social work assessments and IDT (Interdisciplinary Team) participation.
- Ensure timely and effective delivery of social work interventions based on participant care plans and urgent needs.
- Maintain compliance with documentation, assessments, care plan completion, reviews, and participant rights.
- Collaborate with Behavioral Health Specialists to manage programs such as support groups, counseling, and family education.
- Partner with community professionals when needed.
- Provide support in end-of-life care coordination.
- Conduct home visits as required.
Job Requirements:
- Master's degree in Social Work ( MSW) required.
- Social Work licensure preferred.
- At least 2 years of supervisory or leadership experience in social work strongly preferred.
- Minimum 1 year of experience working with frail or elderly populations.
- Strong clinical, organizational, and communication skills.
- Valid driver's license with clean DMV record.
- Proof of current vehicle insurance required.
