Job Title: Social Worker

Location: Beverly, MA 01915.

Work Arrangement: On-site.

Employment Type: Contract.

Duration: 13 Weeks (Extendable).

Schedule: 8 hr. days with weekends (8 am to 4:30 pm).

Pay Rate: $2,500/week to $2,700/week.

POSITION OVERVIEW:

• Medical social work experience, including case management and discharge planning, required.
• Master's degree in Social Work with Current MA Social Work licensure (LCSW/LICSW).
• A minimum of one year of related experience in a hospital environment, with transferable skills that apply to a fast-paced and high-urgency setting.
• Ability to triage crises and collaborate/coordinate with collateral agencies.
• Experience with protective services, child welfare, long-term care, crisis intervention, chronic illness, hospice care, substance use disorders, and psychiatric illnesses is a plus.
• You will be an integral part of the care coordination team, working in collaboration with RN case management, the clinical team, and internal collaterals.
• As a member of our interdisciplinary team, you will be responsible for performing psychosocial assessments, providing supportive counseling, and care coordination with respect to discharge planning and service coordination for adult, geriatric, pediatric, and med/surg populations.
