    Summary:

    The Bachelor of Social Work (BSW) Case Manager plays a pivotal role in maintaining the quality of care patients receive during medical center hospitalization and post dischargewhile ensuring the organizationis fully reimbursed. The BSW Case Manager (CM) must have a highlevel ofexpertiseand skill to create highlyeffective strategies for short term and long term goals in order to prevent readmissions and addressunpredictable situations. BSW CM must be proficient inplanning,executing and monitoring effectivenessof the care provided to ensure timely transition through the continuum of care. Educating and consulting with the physicianand the health care team to ensure timely and appropriatelevel of care isachieved. This requires effective skills in conflict resolution, decision making and team building. The BSW Case Manager practicesin accordancewith the care management processutilizing tools, standards, models, goals and objectives,and performanceimprovement concepts. The BSW Case Manager provides pertinent clinical data to outside agenciesasneeded to assure compliance with their requirements and represents the organizationprofessionally. The ability to analyze,evaluate and distribute resources and educational information to ensure understandingof services to alleducational level is key to the role. The social workeris responsible for assessing the patient’spsychosocial-spiritualneeds, educationand discharge planning needs.

    Responsibilities:

    (30%)* Provide social work interventionas it relates to hospitalizationincluding crisis management, health care decision making, illnessadjustment,ethical/legal concerns, dischargeplanning,and transitional care needs, child or elderabuse, domestic violence, competency, financial problems, compliances issues, substance abuse, mental illnessaswell as otherpsychosocialbarriers to maximizing health status.

     (30%)* Work with health care team, patient, family and/or significant others to continually transition the patient to the appropriate level/placeof care. Advocate, mediate, and negotiate,with an emphasison self-determination for the patient system, to formulate a cohesive plan for maintaining the patient’shealth status, improving social supports, and moving the patient safely intoless restrictive, less costly levels of care according to available resources.

     (20%)*Collaboratewith the physicianin competency/capacity determination,obtaininglegal guardianship,involuntary psychiatricadmission,adoptions,ethical concerns, etc.Completeand disseminate all necessary legal and clinical documentationas needed for resolution.

    (20%)*Collaborateswith all members of the Care Management Team includingphysicians,nurses,healthinformation analysts,andothers to provide information relating to admission, continued stay, discharge criteria, managed care, federal program regulations, reimbursement fundamentals, and regulatory standards on a dailybasis.

     

    OTHER DUTIESANDRESPONSIBILITIES:

    • Complete and document a standard social work psychosocial-spiritualassessment to: Eliminate barriers to treatment and discharge, increase patient/family satisfaction, improve appropriate utilizationof resources. 
    • Serve as the lead in obtaining financial and other resources for patientsand families in need. 
    • Identify the need for and conduct family meetings, with or without the physician, that result inunderstanding, comfort, decision-making, andotherimportant outcomes, such as discharge. 
    • Integrate relevant theories of family dynamics, crisis intervention, strengths based, solution focused, and meaning of illnessinto everyday casework. 
    • Summarize a patientand family situation concisely, with helpful guidance to non-socialwork staff members about recommendations for actions to be taken andbarriers to discharge. 
    • Facilitates discharge planning for patients i.e.extended care placements, other facility transfers, andhome care arrangementsin collaboration with the health care team. 
    • Actively participate in care coordination efforts to successfully identify high-risk factors and respond appropriately so that key information,next steps and avoidabledaysare captured and documented. Is anactiveparticipantin the daily care coordination meetings. 
    • Accepts accountability for the clinical outcomes that the Discharge Planning role can facilitate. 
    • Maintains timely, clear and concise documentation inall required systems. 
    • Provides pertinent clinical data to designatedoutsideagencies to assure compliance with their requirements.
    • Assures compliance with regulatory requirements and acts a resource/liaison to physiciansand medical staff regarding all aspects ofCase Management activities.
    • DuringCare CoordinationRounds, BSW CM communicates with the health care team a patient-centeredplan that is needs based,appropriate to patient resources, timely and addresses the risk of readmission.
    • Builds professional relationships with patient, families, facilities and resources to address conflict resolution with positiveoutcomes.
    • Completesall documentationanddata entry requirements. Identifies high risk populationand addressesopportunities for intervention. 
    • Willingly and consistently offer to helppeers asneeded. 
    • Communicates teamwork, caring and compassion. Provide excellent customer service with internalandexternal customers. 
    • Assume responsibility for professionaldevelopment and education requirements for maintenance ofprofessionallicensure. 
    • All other duties asassigned.
    Certifications and Licensures:

    Credential: BSW: Bachelors of Social Work
    Equivalent Experience: LLBSW or LBSW Required at time of hire. Limited License BSW employees must be actively working to obtain full licensure either as a LBSW or working towards their LMSW. These employees must pass licensing exam and complete 4,000 hours of supervised post-degree social work by a LMSW. Certification in Case Management preferred. Recommended that the Case Manager receives their ACM certification after two years of experience.

    EXPERIENCE, TRAINING AND SKILLS:

    Training and experience,and/ordemonstrated competency in casework principles,includingdiagnosis,assessment, crisis intervention, treatment and transition planning; techniques in individual, family andgroup therapy; awarenessof community resources, public assistanceand entitlementprograms,ensuring transition to appropriatelevels of care; and issues relating to age and stage ofdevelopment, special needsand cultural patternsof the patientpopulationsbeing served.

     

    Knowledge of medical social work practices,processesandproceduresas normallyacquired through three years’ experienceworkingin a medical center (preferred), social agency or community organizationdealingwith physicaland/or mental health and/or welfare.

     

    Interpersonal skills needed to communicate successfully with individualsandgroups and interactwithpeople at all levels to communicate ideas and concepts in a clear and understandable manner.

     

    Adherence to the National Association of Social Workers’ Codeof Ethics.

    Is able to move freely about facility with or without an assisteddevice and must be able to perform the functions of the job as outlinedin the job description.

     

    Overall vision and hearingis necessarywith orwithoutassisteddevice(s).

     

    Frequently required to sit/stand/walk for longperiodsof time. May require frequent postural changes such as stooping, kneeling or crouching.

     

    Some exposure to bloodborne pathogensand otherpotentiallyinfectious material. Must follow MyMichigan Health bloodbornepathogenand TB testing as required.

     

    Ability to handle multiple tasks, getalong with others, work independently, regular and predictableattendance andability to stay awake.

     

    Overall dexterityis required includinghandling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.

     

    Physical Demand Level: Sedentary. Must beable to occasionally (0-33% of the workday)liftor carry 0-10lbs.
