Job Description:

This position is responsible for providing and coordinating social services at the City of Charleston Hope Center, serving as the first point of contact in assessing visitors' social and mental health needs while delivering semi-routine and diversified social work services.

Examples of Duties:

  • Conduct intake assessments to determine client needs and create individualized housing plans.

  • Formulate hypotheses about client challenges to develop appropriate service and treatment plans.

  • Facilitate appointment scheduling with external agencies for services unavailable at the center.

  • Present client assessments and recommendations to appropriate staff or agencies.

  • Participate in group counseling sessions to ensure proper care delivery.

  • Gather information about social problems to identify potential service providers.

  • Monitor client progress in collaboration with professional partners.

  • Maintain demographic reports on client services for stakeholder communication.

  • Coordinate service planning with professional personnel and partnering organizations.

  • Maintain data to ensure reporting for the purposes of the community's knowledge and to pursue grant funding in support of services.

  • Additional duties as assigned.

Basic Qualifications:

  • Bachelor's Degree with 3 - 5 years of experience

  • Or an equivalent combination of education and experience

  • Master's degree in social work preferred

  • Licensed Social Worker through South Carolina Labor, Licensing and Regulation (SC LLR) preferred
