This full-time role supports rising patient volumes and complexity by facilitating goals-of-care conversations, advance care planning, caregiver support, and connections to community resources such as hospice, while helping reduce avoidable ED visits, improve transitions of care, and enhance delivery of goal‑concordant, whole‑person care.
