Education: Master's degree in Social Work required.
Experience: Experience working with the population served by the clinic is highly preferred. Experience providing individual, group, and family therapy, conducting psychosocial assessments, treatment planning, and care coordination is strongly desired.
Certifications/Licenses: Licensed by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with current valid registration. Must be eligible for full and unconditional participation in Medicaid and Medicare programs. May be required to possess a valid New York State driver's license and maintain a satisfactory driving record.

About the Role
The Social Worker establishes and maintains therapeutic relationships with individuals and their families through comprehensive assessment, treatment planning, counseling, advocacy, and care coordination. This role provides individual, group, and family therapy services while collaborating with multidisciplinary treatment teams to ensure clients receive high-quality, person-centered care. The Social Worker serves as a key advocate for clients' clinical, social, and community support needs and helps facilitate successful treatment outcomes and continuity of care.
Primary Responsibilities
Conduct comprehensive psychosocial assessments through record reviews, interviews, and collateral information gathering.
Assess treatment needs and develop clinical recommendations for therapeutic interventions.
Participate in the development, implementation, review, and modification of individualized treatment plans.
Coordinate referrals for ancillary, support, and consultative services and ensure continuity of care.
Provide individual therapy to enhance clients' social, emotional, and behavioral functioning.
Educate clients and families regarding diagnoses, treatment options, community resources, and available services.
Identify and coordinate financial, social service, and community support resources as needed.
Provide crisis intervention services and consultation in a variety of treatment settings.
Assess discharge readiness and develop comprehensive discharge and aftercare plans.
Prepare timely progress notes, treatment recommendations, reports, and other required documentation.
Communicate relevant clinical information to treatment team members and service providers.
Facilitate life-skills training groups, counseling groups, and psychoeducational sessions.
Develop group curricula and ensure sessions align with treatment goals and participant needs.
Document attendance, participation, and outcomes of group services.
Evaluate group effectiveness and implement improvements as needed.
Provide family counseling, education, support, and referrals to strengthen treatment outcomes.
Promote and increase family and support system involvement in treatment.
Facilitate family education sessions and support engagement in recovery and treatment planning.
Collaborate with community providers and service systems to ensure coordinated care.
Supervise social work students when assigned, including orientation, training, and clinical oversight.
Perform administrative and supervisory duties as assigned, including staff guidance, scheduling, and performance monitoring.
Perform other related duties as assigned.
