SOCIAL WORKER

Conducts ongoing clinical assessments for competency, stability, and level of functioning; determines primary issues by obtaining mental health, medical, substance abuse and cognitive histories; determines available and needed resources, including housing, family support, work history, social security, Medicaid and Medicaid waivers, etc.; obtains release of information for records and collaboration with community providers; interviews family members for collateral information; writes case files and narrative reports; and meets and discusses clinical recommendations with attorneys. Processes referrals for mental health counseling, substance abuse treatment, developmental disabilities and other services; reviews, completes and processes related applications, waivers and documentation; assists clients with completing and submitting required forms and documentation for services; contacts providers and coordinates interviews with clients at the jail; provides medical records required for placement; coordinates service provision and placements including, vocational rehabilitation services, nursing homes, assisted living placements, job training, and education assistance. Maintains a comprehensive, current knowledge of applicable policies, procedures, regulations, codes, and other regulatory standards related to aging services; maintains and awareness of available community services and resources; and attends workshops, seminars, and training sessions as appropriate. Performs other related duties as required. Master"s degree in social work or related field required; supplemented by three years of experience in mental health and substance abuse evaluation, case management, service coordination, or related field; or any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities for this job. Must possess and maintain a valid South Carolina Driver"s License. NOTE: SALARY IS SCALABLE BASED OFF EXPERIENCE AND OTHER FACTORS.
