Position Overview

We are seeking an experienced Licensed Clinical Social Worker (LCSW) to support care management, discharge planning, and care coordination in an acute care hospital setting.

Required Qualifications

• Master of Social Work (MSW) from an accredited institution.

• Active Washington State license appropriate for the position; pending licenses are not accepted.

• Minimum 1 year of relevant professional experience.

• Previous acute care hospital experience.

• Previous travel healthcare assignment experience.

• Demonstrated experience in discharge planning and care coordination.

• Current Basic Life Support (BLS) certification.

• Experience using Epic electronic medical records.

• Strong knowledge of psychosocial assessments, care transitions, and post-acute service coordination.

• Excellent communication, documentation, organization, and problem-solving skills.

• Ability to work independently and collaboratively with multidisciplinary healthcare teams.

• Willingness to float between hospital departments and both Colby and Pacific campuses.

• Availability to work every other weekend.

Preferred Qualifications

• Experience in Medical-Surgical (Med-Surg), Length of Stay (LLOS), and Emergency Department (ED) settings.

• Diverse case management and discharge planning experience across acute care specialties.

• Familiarity with AIDA electronic medical record systems.

• Experience coordinating complex discharges and working with post-acute care facilities.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify patient needs, social barriers, and discharge planning requirements.

• Coordinate discharge planning and care transitions across acute care hospital departments.

• Collaborate with physicians, nurses, case managers, therapists, and other healthcare professionals to develop patient care plans.

• Assess patients' social, emotional, financial, and environmental needs affecting recovery and discharge readiness.

• Coordinate post-acute care services, including rehabilitation, skilled nursing facilities, home health services, and community resources.

• Facilitate communication among patients, families, caregivers, and multidisciplinary care teams.

• Identify discharge barriers and develop strategies to support timely, safe patient transitions.

• Maintain accurate clinical documentation, assessments, and discharge plans in Epic.

• Support utilization management and care coordination in accordance with hospital policies and applicable regulations.

• Assist with complex discharge planning and resource coordination for patients with multiple medical or psychosocial needs.

• Float between hospital departments and provide coverage at both Colby and Pacific campuses as assigned.

• Collaborate effectively in a fast-paced acute care environment.
