Per diem hours -
JOB TITLE: SOCIAL WORKER
DEPARTMENT: Psychosocial Services/Care Coordination
ACCOUNTABILTY: Nursing Home Administrator
WORK AREAS: McLean Health Center
CLASSIFICATION: Exempt
POSITION SUMMARY: Responsible for the resident management process by collaborating daily with Interdisciplinary team (IDT) members involved in the care of residents including nursing staff, rehab, admissions, medical team and Administrator. Identifies areas needing attention/improvement, makes recommendations and directs interventions to facilitate change. Functions as resident liaison to IDT and families. As needed, provides social work services to Assisted Living and Independent Living residents on the McLean campus. Responsible to ensure Resident Rights are maintained at all times, especially during care transitions. Liaisons for all room changes within SNF and between SNF and AL/IL and team.

Requires corrected vision and hearing within normal range. Requires the use of office equipment such as telephones, fax machines, computers, and copiers. Requires eye-hand coordination and manual dexterity.
Typical Working Conditions:
Work is primarily performed in an office environment or on the nursing units. Contact with residents, families, and other professionals, as well as all McLean staff.
EMPLOYMENT REQUIREMENTS:
Knowledge, Skills, Abilities:
Knowledgeable and competent in the social work process within skilled nursing and assisted living environments. Knowledge of a wide range of disabilities and QI. Strong organizational and communication skills. Skill in establishing and maintaining positive working relationships with clients, families, and staff. Ability to maintain confidentiality of sensitive information. Ability to adhere to regulatory guidelines
Education:
Bachelor's or Master's degree in Social Work or Bachelor's degree in related field.
Experience:
Minimum of one year social work experience in SNF setting including rehab and long term care, and/or AL experience.
