Come join us at Ozanam Hall Nursing Home of Queens!  A Faith-Based, Nonprofit, Short-Term Rehabilitation and Skilled Geriatric Care facility sponsored by the Carmelite Sisters for the Aged and Infirm, we've served the community since 1971.   We are a place of life, a place of caring, and more importantly, a true home for our residents. 

We are seeking a Social Worker - for a TEMPORARY assignment.

• $40 per hour.
• Full-Time; Monday - Friday schedule.

Social Worker Qualifications:  

• Must have a Bachelor's Degree in Social Work or related field.
• One year of Social Work in a Nursing Home required.
• Must have MDS assessment experience.
• Experience using PCC is a plus.

Social Worker Introduction: 

Under the supervision of the Director of Social Work, assist the residents to attain or maintain the highest practicable physical, mental and psychosocial well-being, and in navigating the health care system.   

Social Worker Job Summary: 

• Interview residents and their families.
• Provide assessments and counseling to residents and families.
• Develop care plans based on the individual need of residents.
• Provide crisis intervention as needed.
• Provide grief and supportive counseling.
• Coordination of discharge planning and case management.
