Pay: USD 35/hr – USD 38/hr

Position summary:
Provides case management and social work services by assessing individuals, developing care plans, coordinating resources and referrals, monitoring rehabilitation progress, advocating for clients, and providing crisis intervention.
Key Responsibilities:

• Assessing individuals, developing individual care plans and determining necessary resources.
• Advocating for and assisting individuals and families in accessing and obtaining services.
• Contacting service providers and attending interdisciplinary conferences to determine appropriate placement and referrals.
• Monitoring individual rehabilitation goals and progress.
• Providing crisis intervention services.   

 
Minimum Qualification:
Bachelor's in Social Work degree, working under the supervision of a licensed master social worker (LMSW) or licensed clinical social worker or New York State LCSW or LCSWs. Valid drivers license.
