Job Description

The Solid Organ Transplant Clinical Pharmacist provides specialized clinical pharmacy services to transplant patients, ensuring safe, effective, and evidence-based medication therapy. This role focuses on interpreting and evaluating complex medication orders, supporting oncology and pediatric acute care needs, and collaborating closely with the healthcare team to optimize patient outcomes in a hospital setting.

Responsibilities

  • Interpret and evaluate medication orders to ensure appropriate drug selection, dosage, route, and duration of therapy for each patient, identifying potential adverse reactions or interactions and contacting the prescribing clinician as needed.
  • Review, fill, and dispense physician medication orders and prescriptions in accordance with hospital policies, professional standards, and regulatory requirements.
  • Verify orders and prescriptions prepared by pharmacy support personnel, ensuring all products are accurately prepared, labeled, and documented before distribution.
  • Serve as a subject matter expert in drug information, providing evidence-based recommendations and guidance to physicians, nurses, and other healthcare professionals.
  • Provide clear and compassionate patient education regarding medication regimens, including indications, dosing, administration, side effects, and monitoring requirements.
  • Complete thorough medication history documentation, obtaining and reconciling information from patients, caregivers, and external sources to support safe prescribing and transitions of care.
  • Perform quality improvement activities within the pharmacy and clinical practice areas, contributing to the development, implementation, and evaluation of initiatives that enhance patient safety and medication use processes.
  • Supervise pharmacy support personnel within the assigned work area, ensuring that medications and related products are prepared, packaged, and distributed in accordance with established procedures, schedules, and protocols.
  • Collaborate with the multidisciplinary transplant and oncology teams to support individualized treatment plans and ensure continuity of care.
  • Maintain accurate, timely, and complete documentation of all clinical interventions, dispensing activities, and patient interactions in accordance with hospital and regulatory standards.

Essential Skills

  • PharmD or Bachelor's degree in Pharmacy plus up to one year of post-degree coursework and/or an internship.
  • Two to three years of general pharmacy experience, or at least one year of directly related pediatric pharmacy experience.
  • Pediatric acute care oncology experience.
  • Hospital pharmacy experience, including exposure to oncology practice.
  • Current license and registration with the Massachusetts Board of Registration in Pharmacy.
  • Strong analytical skills to collect information from diverse sources, apply professional principles to perform complex analyses, and summarize data to solve problems or design systems and programs.
  • Effective communication skills to address conflicting views or issues, mediate fair solutions, and collaborate productively with the healthcare team.
  • Well-developed writing skills for clear documentation, clinical notes, and communication of drug information.
  • Ability to verify and check medications prepared by support personnel with a high level of accuracy and attention to detail.

Additional Skills & Qualifications

  • Doctorate degree in Pharmacy (PharmD) preferred.
  • Experience in oncology pharmacy practice beyond the pediatric acute care setting is highly valued.
  • Demonstrated ability to participate in and contribute to quality improvement initiatives within a hospital or clinical environment.
  • Ability to provide patient-centered education and counseling, particularly in complex transplant and oncology treatment plans.
  • Strong interpersonal skills to work effectively within multidisciplinary teams and to support and supervise pharmacy support staff.

Work Environment

This position is based in a hospital environment with a focus on solid organ transplant and oncology patient care. The schedule is Monday through Friday on an 8-hour shift, primarily during daytime hours, with a rotating weekend assignment approximately every fourth weekend. The role involves close collaboration with physicians, nurses, and other clinical staff, frequent interaction with patients and families, and the use of hospital pharmacy systems and technologies to manage medication orders, documentation, and dispensing. The work setting emphasizes adherence to established procedures, protocols, and safety standards, and requires the ability to manage a steady workflow while maintaining accuracy and clinical rigor.

This is a Permanent position based out of Boston, MA.

Pay and Benefits

The pay range for this position is $150000.00 - $200000.00/yr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
