The Ambulatory Abdominal Transplant Clinical Pharmacist will cover kidney, pancreas, and liver patients in the Abdominal Transplant Clinic which is a multidisciplinary clinic that includes surgical, renal, and hepatology attendings, fellows, and APPs as well as dieticians, social workers, transplant coordinators, and clinical pharmacists. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events.
