    Home Care Intake Specialist

    We are seeking an organized and detail-oriented Intake Specialist for a home care agency. Responsibilities include handling new client referrals, gathering required documentation, completing intake paperwork, verifying eligibility, coordinating with clients and families, and working closely with the clinical and operations teams to ensure a smooth and timely intake process. Previous home care or healthcare intake experience required
