POSITION SUMMARY

The Speech Pathologist is responsible for providing therapy to patients at home or in alternative settings.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Provides direct and indirect therapy services to patients based on a modified primary therapy approach. Such provision of care includes, but is not limited to:

  • Evaluate and diagnose speech, language, communication, and swallowing disorders.
  • Treat speech, language, communication, and swallowing disorders.
  • Patient and caregiver teaching
  • Coordination of related services
  • Development and revision of a therapy plan of care Skilled therapy visits Implementation of physician orders Monitoring of patient response to care provided

Completes all required patient care documentation and reports with accuracy and in a timely manner in accordance to internal and external regulatory requirements.

Communicates with all disciplines on providing care for the patient including nursing staff, other therapy providers and physicians as appropriate to ensure quality and continuity of care.

Organizes daily work activities in accordance with patient needs.

Participates in activities pertaining to the terms of the Performance Improvement Program.

Maintains all professional and regulatory requirements, such as: orientation compliance, clinical competence, professional licensure, certification(s), health requirements, continuing education hours and any other related accountability.

Contributes to the In-services of other speech pathologists as needed.

Attends in-services, departmental meetings and other related discussions as requested.
