We are currently seeking a passionate and skilled Speech Therapist to join our dynamic Rehab team. You\'ll provide individualized, patient-centered therapy services that help clients regain function, improve communication, and maintain independence-all while enjoying a healthy work-life balance.

What You'll Do:

• Evaluate, plan, and implement speech-language pathology treatments in the home setting

• Collaborate with physicians, nurses, and interdisciplinary care teams

• Document progress and adapt care plans based on patient needs

• Educate patients and families to support treatment goals

• Provide compassionate, evidence-based care that promotes dignity and well-being

• Maintain compliance with all state and agency requirements

• Accurately document treatments, standardized tests, patient education, and functional progress in the EMR.

• Ensure documentation reflects visit-to-visit progress or interventions aimed at preventing deterioration.

• Synchronize documentation per agency standards and timelines.

What you Bring:

• Current SLP license in New Hampshire and Maine (or ability to obtain)

• Minimum 1 year of home care experience preferred

• Strong communication and time-management skills

• Passion for making a difference in the lives of patients and families
