About the Position

Harris Health is seeking a Supervising Pharmacy Transition Specialist to support safe and seamless medication management for patients preparing to leave Ben Taub Hospital.

In this role, you will actively engage patients, reconcile discharge medications, fill prescriptions, coordinate bedside delivery, and provide medication education before discharge. You will also make follow-up calls to reinforce education, answer medication-related questions, and help ensure patients continue appropriate therapy after leaving the hospital.

The Pharmacy Transition Specialist works closely with pharmacists, pharmacy technicians, nurses, physicians, and other clinical partners to ensure prescriptions are accurate, safe, delivered on time, and clearly understood. This role also oversees Pharmacy Technician Transition Specialists and supports inpatient pharmacy operations when needed.

Requirements

  • Education
    • Bachelor’s Degree in Pharmacy or PharmD from an accredited School of Pharmacy required
    • Doctor of Pharmacy preferred
  • Licensure and Certifications
    • Current Texas Pharmacist License or eligibility to obtain licensure within 90 days of employment
    • Basic Life Support certification from the American Heart Association or Red Cross within 90 days of employment
    • IV Sterile Certification required for inpatient areas and Fondren Pharmacy at Smith Clinic
    • Board of Pharmacy Specialties certification preferred
    • Texas Pharmacist Preceptor Certification preferred
  • Experience
    • Minimum of 2 years of related pharmacy experience or completion of a PGY1 residency
  • Language Skills
    • Bilingual Spanish preferred
  • Communication Skills
    • Strong verbal and written communication
    • Exceptional verbal communication and public-speaking skills
    • Ability to explain medication information clearly to patients and families
