Counseling Center is seeking a full-time experienced Adult/Child Targeted Case Manager to coordinate efforts to ensure that persons served receive holistic service planning, continuity of care, effective services, and ongoing advocacy in accordance with Medicaid documentation requirements and Center policy and procedures.
Targeted Case Managers coordinate the development of individualized, comprehensive service plans, which allow persons served the opportunity to achieve their goals and optimize their level of independence.
Targeted Case Managers act as advocates for persons served; they link persons served with needed services; monitor the persons served progress and provide individualized support. Targeted Case Management services and activities should be provided in community locations which may include persons-served residences, medical and behavioral health sites, hospitals, community resource sites, shelters, and jails.
Responsibilities: intakes, assessments, service plans. Provides resources, home visits, referrals, education, prevention, and intervention services. Assures adequate and complete documentation of service plans, progress notes, billing, and all other required documentation for assigned persons served. Coordinates needed ancillary services with other staff, physicians, agencies, and facilities. Attends various meetings, staffing, and workshops. Ensures follow-up of all persons served. Maintain an expected level of productivity.
Education/Experience Requirements: Must have the following certification, Core Competency, and two of the following: Substance Use Disorder, Severe Mental Illness, Co-Occurring/ Physical Health, Severe Emotional Disturbance Case Manager Certifications through the Kentucky Certification Board. A bachelor's degree in Human Services, Psychology, Social Work, or a related human services field is required and one full year of full-time or equivalent experience in working with children/adults who have a serious mental illness; or a bachelor's degree from an accredited university or college is required and three years full-time experience working with children/adults with serious mental illness or co-occurring Conditions. Must have Targeted Case Manager experience AND certification.
