RN CARE MANAGER – HOSPICE.
SHIFT: DAYS.
Schedule: Monday–Friday.
Hours: 8:30 AM – 5:00 PM.
Mileage Reimbursement: Available.
JOB DESCRIPTION:
The RN Care Manager provides compassionate, high-quality nursing care to hospice patients and their families. This role requires strong bedside nursing skills, particularly in symptom management, crisis intervention, and family education.
KEY RESPONSIBILITIES:
- Manage patient care and provide specialized nursing services to hospice patients.
- Assess and manage patient symptoms and respond to care-related crises.
- Educate patients and families on care plans and symptom management.
- Develop and maintain positive relationships with patients and families.
- Collaborate with the interdisciplinary healthcare team to ensure quality patient care.
- Maintain effective written and verbal communication and demonstrate strong problem-solving and decision-making skills.
- Provide care primarily in patients’ homes, with travel to long-term care facilities, hospitals, and other locations within the service area.
REQUIREMENTS:
- Graduate of an accredited nursing school – Required.
- Active Virginia RN license, verified through Nursys – Required.
- Minimum of 2 years of experience in acute care, home health, hospice, medical-surgical, or geriatric nursing – Required.
- Current Virginia driver's license – Required.
- Driving record must meet applicable policy standards – Required.
- BLS (AHA) certification – Required.
