About the RoleAs a THL Care Coordinator, you will provide therapeutic interventions, case coordination, and resource linkage to clients and other involved parties (i.e., family, DCS, Court personnel, non-relative supports, etc.) to support identified changes within the individual or family. This role focuses on delivering client-centered services and collaborating with stakeholders to improve outcomes for children, adolescents, and families.
Responsibilities· Provide therapeutic interventions to children and adolescents in a community-based treatment environment· Coordinate care and services for assigned clients· Link clients and families to appropriate community resources· Collaborate with family members, DCS, court personnel, and non-relative supports· Support identified behavioral and emotional goals for clients· Maintain accurate and timely documentation in accordance with organizational and regulatory standards· Participate in team meetings and case reviews as required· Ensure services are delivered in a competent, ethical, and client-centered manner
Required Qualifications· Bachelor’s degree in a Human Service discipline from an accredited four-year college or university· Minimum of 1 year of experience working with children and adolescents in a therapeutic, community-based treatment environment
Preferred Qualifications· Experience coordinating care with multiple systems including child welfare and court systems· Strong communication and collaboration skills
