    Transition Coordinator

    Department: Pupil Personnel Services

    Position Summary

    The Public Schools of the Tarrytowns are seeking an innovative, collaborative, and student-centered Transition Coordinator to lead and expand transition services for students with disabilities in grades 6-12 as well as students participating in post-secondary transition programming. The Transition Coordinator will develop meaningful opportunities that prepare students for employment, post-secondary education, independentliving, and active participation in their communities.

    This position will serve as the district's primary liaison between students, families, schools, community agencies, businesses, and adult-service providers to ensure seamless planning and successful outcomes beyond high school. Bilingual (Spanish) strongly encouraged to apply.

    Primary Responsibilities

    The Transition Coordinator will:

    • Develop, coordinate, and expand Community-Based Instruction (CBI), internships, job shadowing, vocational training, and paid and unpaid work experiences for students with disabilities.

    • Build and maintain partnerships with local businesses, colleges, trade schools, community organizations, and adult-service agencies to increase post-secondary opportunities for students.

    • Collaborate with special education teachers, school counselors, administrators, related service providers, and families to develop high-quality, individualized transition plans aligned with each student's strengths, interests, and goals.

    • Serve as the district liaison between schools, employers, community organizations, families, and adult-service agencies including OPWDD, ACCES-VR, Care Coordination Organizations (CCOs), and other community supports.

    • Assist families in navigating eligibility requirements, applications, referrals, and timelines for adult-service agencies and post-secondary supports.

    • Support students and families in exploring college programs, vocational and technical education, supported employment, day habilitation programs, independent living opportunities, and other post-secondary pathways.

    • Coordinate transition activities across schools to ensure continuity of services and successful planning beginning no later than age 15.

    • Monitor transition goals, student outcomes, and program effectiveness while ensuring compliance with IDEA and New York State transition requirements.

    • Promote student self-determination, self-advocacy, independence, career readiness, and workplace skills.

    • Organize transition fairs, family workshops, employer engagement events, and informational sessions related to post-secondary planning and adult services.

    • Collaborate with district administrators to develop strategic initiatives that expand transition opportunities and strengthen partnerships throughout the community.

    • Maintain accurate records, data, and reports related to transition services and outcomes.

    • Stay current on best practices, legislation, and resources related to transition services and special education.

    Qualifications

    • New York State certification in Special Education, School Counseling, School Social Work, School Psychology, or a related field.

    • Minimum of five years of successful experience working with students with disabilities.

    • Demonstrated knowledge of IDEA transition requirements, New York State special education regulations, and transition planning.

    • Experience working with adult-service agencies such as OPWDD, ACCES-VR, and Care Coordination Organizations.

    • Strong organizational, communication, and collaboration skills.

    • Experience developing community partnerships and coordinating work-based learning opportunities preferred.

    • Knowledge of Community-Based Instruction and evidence-based transition practices preferred

    SALARY: Per TAT Contract
