As a Case Manager RN, you will be responsible for coordinating patient care, ensuring quality outcomes, and facilitating communication among healthcare providers. You will need to have at least 2 years of acute care experience, and first-timers are welcome with clinical approval. This position requires you to work every other weekend and may require floating between like units based on need.
