Shift: Days | 5x8 | 8:00 AM – 4:30 PM
Weekend: Every Other Weekend
Requirements:
- Minimum 2 years of acute hospital Case Management experience.
- Must have recent acute care Case Management experience within the last year.
- Experience in care coordination and discharge planning.
- Epic experience required.
- Previous travel experience required; first-time travelers may be screened with Clinical approval.
- Experience with HMOs, IPAs, etc.
- Experience with Advance Directives, POLST, and protective services processes.
- Experience with admission criteria, utilization review, appeals/denials, benefits eligibility, and DME needs assessment.
- Acute hospital experience across ED, ICU, MS, Pediatrics, and SDU/PCU/IMC/Observation preferred.
