Description

American Traveler is hiring an RN Case Manager for a Level III trauma center requiring 2 years of experience in case management and Epic charting proficiency.

Details

  • Case Management unit within a short-term acute care Level III trauma center with 252 staffed beds
  • Patient population includes med/surg, neurology, oncology, cardiac, and OBS patients
  • Day shift schedule, 5x8 hours, 8:00 AM to 4:30 PM
  • Required to work every other weekend
  • Part of the Yale New Haven Health System
  • Free parking available

Requirements

  • Active RN license (CT or Compact) required
  • 2 years of case management experience required
  • Acute hospital and home health experience required
  • Epic charting system experience required
  • Travel experience required
  • 1 professional reference required

Additional Information

  • Coordinate quality, cost-effective patient care across multidisciplinary teams
  • Perform biopsychosocial assessments and develop individualized care plans
  • Facilitate discharge planning and continuity of care
  • Educate patients and families to support care transitions
  • Flu vaccine required with no declinations accepted
  • COVID vaccine and booster required with medical or religious exemption only
  • Must reside outside a 75-mile radius of the hospital to be eligible for travel rate
  • Returning staff must have a 2-year gap in employment to be considered as a traveler
  • Up to 7 days of RTO can be auto-approved
