Description

American Traveler is hiring a Field RN for a Home Health position in North San Diego, requiring a CA RN license and prior home health experience with OASIS and HCHB expertise.

Details

Field-based Home Health position covering the North San Diego territory
Geriatric adult home health patient population
Average caseload of 5-7 patients per day
Schedule is Monday-Friday, day shift (8:30am-5:00pm), with one required weekend per month
No on-call responsibilities
Coverage area is within a 50-mile radius of the hub office
Mileage between patient homes reimbursed at the IRS standard rate
Uses HomeCare HomeBase (HCHB) EMR system

Requirements

Active CA RN license required in hand
BLS certification required
Minimum 1 year of Home Health experience required
OASIS experience required
HCHB charting system experience required
Previous charge experience preferred
Experience in adult home health and home infusion settings
Skilled in clinical assessment documentation, infection control precautions, vital signs, respiratory care (nebulizer utilization/teaching, oxygen safety/administration), and phlebotomy/IV management

Additional Information

Responsibilities include admission assessments, case management, discharge planning, recertification, resumption of care, routine visits, and monitoring for significant changes in condition
Coordinate services and maintain documentation in compliance with home health guidelines
Local candidates accepted at the travel rate
