Description

American Traveler is hiring a Home Health RN for a 13-week field assignment in North San Diego, requiring CA RN license, BLS, and prior home health experience with HCHB and OASIS proficiency.

Details

• Field-based Home Health position covering the North San Diego territory within a 50-mile radius of the office hub
• Geriatric patient population with an average caseload of 5-7 patients per day
• Schedule is Monday-Friday, 5x8-hour day shifts (8:30 AM-5:00 PM), with one weekend per month required and schedule adjusted accordingly that week
• No on-call responsibilities
• Uses HomeCare HomeBase (HCHB) as the charting/EMR system
• Mileage reimbursed at the IRS standard rate between patient homes

Requirements

• Active CA RN license required in hand (pending licenses not accepted)
• BLS certification required
• Minimum 1 year of Home Health experience required
• HCHB charting system experience required
• OASIS experience required
• Skills include clinical assessment documentation, care coordination, infection control, vital signs, respiratory care, nebulizer and oxygen administration, and phlebotomy/IV management

Additional Information

• Responsibilities include admission assessments, case management, discharge planning, recertifications, resumption of care, routine visits, and documentation of significant changes in condition
• Works within adult home health and home infusion settings
