The LPN will provide direct patient care, administer medications and treatments, monitor resident conditions, and collaborate with the interdisciplinary care team to deliver exceptional patient-centered care.

COMPENSATION- Pay Rate:$33.48 - $39.06 per hour
KEY RESPONSIBILITIES- Administer medications and treatments as prescribed.
- Monitor and document resident conditions and responses to care.
- Provide direct nursing care and support activities of daily living.
- Collaborate with physicians, nurses, therapists, and other healthcare professionals.
- Maintain accurate and timely nursing documentation.
- Respond promptly to changes in resident condition.
- Ensure compliance with clinical standards and regulatory requirements.
ADDITIONAL INFORMATION- Skilled Nursing and Long-Term Care setting.
- 12-hour day shift assignment.
- 1-hour meal break included.
- Agency staff must have completed the original or updated COVID-19 vaccination series or provide a documented religious and/or medical exemption where applicable.
- Additional vaccine doses must be obtained where required by law.
- Registry personnel must bring a state or federal issued identification and wear a photo badge at all times.
CERTIFICATIONS & LICENSURE- Active Licensed Practical Nurse (LPN/LVN) License
