The LPN will provide direct patient care, administer medications, monitor resident conditions, and collaborate with the interdisciplinary care team to ensure exceptional patient outcomes.

GUARANTEED HOURS- 24 Hours Per Week
- Pay Rate:$30.60 - $35.70 per hour
KEY RESPONSIBILITIES- Administer medications and treatments according to physician orders.
- Monitor resident health status and report changes in condition.
- Maintain accurate and timely nursing documentation.
- Assist with care planning and coordination of services.
- Support activities of daily living and promote resident comfort and safety.
- Collaborate with physicians, nurses, therapists, and healthcare staff.
- Ensure compliance with healthcare regulations and nursing standards.
ADDITIONAL INFORMATION- Skilled Nursing and Long-Term Care setting.
- Night shift assignment.
- Every Other Weekend Required.
- Schedule consists of 2 shifts one week and 4 shifts the following week.
- 30-minute meal break included.
- Nurses wear navy blue scrubs.
- Appropriate work shoes are required.
- Crocs are not permitted per facility policy.
CERTIFICATIONS & LICENSURE- Active Licensed Practical Nurse (LPN/LVN) License
