Job Summary:
Long Term Care (LTC) Nurse Practitioner position is available in Medford, Wisconsin. This role involves providing comprehensive nursing care within a skilled nursing setting, supporting short-term and long-term residents, and functioning across multiple units. The position offers a mix of day shifts with every other weekend and holiday rotations, emphasizing medication administration and patient care.

Job Details:

  • Location: Medford, WI
  • Shift: Days, 8 hours per shift
  • Schedule: 40 hours per week
  • Start Date: 08/17/2026
  • Required Qualifications: WI Registered Nurse (RN) license, BLS certification (American Heart Association or ARC), minimum 2+ years of previous experience in skilled nursing long-term care setting, and associate degree or certificate.
  • Preferred Qualifications: Bachelor's degree in Nursing.
  • Float: Float rotation may be required to short-term, long-term, and community-based residential facility units.
  • On-Call/Call Back: On-call for low census hours; call back pay applicable with a minimum of 2 hours.
  • Additional Information:
    • Two units: Unit 1 with 12 beds for short-term residents (average census 5-8), and 10 CBRF residents; Unit 2 with 20 beds for long-term residents.
    • Staffing: 3 aides and 2 nurses per shift during day and evening shifts.
    • Scrubs: Any clean, work-appropriate scrubs are acceptable.
    • Flu season policy: Declination of flu and COVID-19 vaccinations requires wearing a mask at all times, with no exemptions.
