Travel LPN – Skilled Nursing Facility (SNF)

Location: Amsterdam, NY
Schedule: 5×8-hour shifts | Must be available for at least 4×8-hour shifts
Shift: Evening – 2:00 PM–10:30 PM
Every Other Weekend Required
Holiday Availability Required

About the Opportunity

A skilled nursing and rehabilitation facility in Amsterdam, NY is seeking an experienced Travel Licensed Practical Nurse (LPN) to join its team on a contract basis. This is an excellent opportunity for an experienced SNF/LTC nurse who is comfortable working independently during the evening shift and has prior travel nursing experience.

Responsibilities

• Provide high-quality nursing care to residents in a skilled nursing/long-term care setting.

• Monitor residents' conditions and report changes to the RN, physician, or appropriate clinical leadership.

• Administer medications and treatments in accordance with physician orders and facility policies.

• Perform routine nursing procedures and provide treatments as assigned.

• Assist with admissions, transfers, and ongoing resident care.

• Accurately document resident care using MatrixCare.

• Communicate effectively with residents, families, physicians, RNs, and the interdisciplinary care team.

• Follow individualized care plans and facility policies.

• Respond appropriately to changes in resident condition during evening hours.

• Maintain compliance with facility policies, NYS regulations, and applicable standards of care.

Requirements

• Active New York State LPN license required.

• BLS certification required.

• Minimum 2 years of LPN experience in an SNF/LTC setting.

• Previous travel/contract nursing experience required, preferably with demonstrated contract renewals.

• Experience with MatrixCare charting preferred/required.

• Must be willing and able to work evening shifts, weekends, and holidays.

• Must be available to work every other weekend.

• Must be able to commit to at least four 8-hour shifts per week.

• Strong clinical judgment, communication, and time-management skills required.

Important Facility Requirements

• 2:00 PM–10:30 PM evening shifts only.

• Candidates must be willing and able to work holidays.

• Every other weekend is required.

• The facility will not consider former employees as travelers; previous staff members are not eligible for this travel/contract opportunity.

Ideal Candidate

The ideal candidate is an experienced LTC/SNF LPN with strong clinical skills who can work independently during evening hours. Candidates with prior travel nursing experience, successful contract renewals, and familiarity with MatrixCare are strongly preferred.
