Job Description – Pediatric Case Manager Registered Nurse (RN)

The Pediatric Case Manager Registered Nurse (RN) is responsible for coordinating and managing the care of pediatric patients throughout their hospital stay. The RN collaborates with physicians, families, social services, and the interdisciplinary healthcare team to ensure patients receive appropriate, cost-effective, and timely care while facilitating safe discharge planning and continuity of care.

Responsibilities:

  • Coordinate and manage the care of pediatric patients from admission through discharge.
  • Perform comprehensive patient assessments and develop individualized case management plans.
  • Collaborate with physicians, nursing staff, therapists, social workers, and community agencies to coordinate patient care.
  • Facilitate discharge planning, including home health services, durable medical equipment, rehabilitation, and follow-up appointments.
  • Communicate with insurance providers regarding authorizations and utilization management as needed.
  • Monitor patient progress and identify barriers to discharge or continuity of care.
  • Advocate for patients and families while ensuring quality outcomes and efficient resource utilization.
  • Document all case management activities accurately in the Epic electronic medical record.
  • Float within scope of practice to support facility needs as required.

Qualifications:

  • Minimum 2 years of RN experience.
  • Pediatric nursing experience required.
  • Acute Care experience required.
  • Epic EMR experience required.
  • First-time travelers accepted.
  • Active Alaska RN License required for Auto Offer (pending licenses may be screened but must be in hand before an offer can be issued).
  • ACM (Accredited Case Manager) Certification preferred.

Assignment Details:

  • 13-week travel assignment
  • ASAP start
  • 5×8-hour Day shifts
  • Shift times may vary:
    • 7:00 AM – 3:30 PM
    • 7:30 AM – 4:00 PM
    • 8:00 AM – 4:30 PM
    • 8:30 AM – 5:00 PM
  • Every other weekend or four weekend days in a four-week schedule
  • No on-call
  • Guaranteed 72 hours every two weeks
  • Travelers may be called off for one 8-hour shift per pay period
  • Floating within scope of practice may be required
  • Block scheduling not available
  • Travelers must reside at least 50 miles from the facility
  • Only one time-off request (up to 5 days) is permitted during the 13-week assignment
  • No holiday time-off requests accepted for Auto Offer
