Start Date: 11/10/2025 | End Date: 02/08/2026
Shift: 8:00 AM – 4:30 PM | Assignment Type: Long-Term (13 weeks)

Overview:
Ascension St. Vincent is seeking an experienced Registered Nurse Case Manager to coordinate interdisciplinary care plans for patients from admission through discharge within an acute care hospital setting.

Responsibilities:

  • Develop, coordinate, and evaluate comprehensive care plans in collaboration with healthcare teams.

  • Assess patient needs, monitor medical necessity, and ensure appropriate level of care.

  • Facilitate care transitions and advocate for resources to remove barriers to effective discharge planning.

  • Collaborate with physicians, social workers, and care transition teams to optimize patient outcomes.

  • Educate staff on documentation standards, level-of-care criteria, and discharge resources.

Requirements:

  • Licensure: Active RN license (IN or compact).

  • Certifications: BLS required.

  • Experience: Minimum 3 years as an RN, with at least 2 years of case management experience in an acute care hospital.

  • EMR/Systems: Familiarity with Ascension systems and documentation workflows preferred.

Additional Details:

  • Start: ASAP preferred

  • Must complete Ascension onboarding, credentialing, and health compliance requirements prior to start.
