Job Title: RN Case Management Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum 2 years of recent RN Case Management experience, with experience in a Long-Term Acute Care or Specialty Client setting. License: Current RN license. Certifications: BLS certification through the American Heart Association. Must-Have: Strong interdisciplinary team collaboration; experience in discharge planning across acute care settings; knowledge of MCG guidelines. Description: Coordinates management of care and ensures optimum utilization of resources. Monitors service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment and care planning. Implements care plans and evaluates individual patient needs. Enhances the quality of patient management and satisfaction. Promotes continuity of care and cost-effectiveness through integration of case management functions. Utilizes discharge planning to address the holistic needs of the patient. Serves as a patient advocate through effective resource utilization.
