Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (07:30 - 16:00) Experience: Minimum of 1 year License: RN License in hand required Certifications: BLS preferred, ACM Certification preferred Must-Have: - Epic experience - Acute care experience Description: The Case Manager RN will be responsible for discharge planning. The candidate must have an active RN license in hand for an auto offer. Shifts may vary but will typically be during daytime hours. Weekend shifts are required every other weekend or four weekend days within a four-week schedule. The nurse may float within case manager services to cover staffing gaps as needed. First-timers in case management are accepted for this role. The role does not require on-call duty. The candidate must have experience in an acute care hospital setting, as behavioral health experience does not apply. Knowledge of charting systems, specifically Epic, is required. The nurse will collaborate with interdisciplinary teams to provide comprehensive patient care. The patient population includes a range of age groups from neonates to geriatrics. The daily census includes a high volume of patients, requiring effective coordination of care. The nurse will utilize established criteria for determining medical necessity and must have a solid understanding of CMS guidelines. Candidates should be willing to work two out of the three major holidays: Thanksgiving, Christmas, and New Year. Additional responsibilities may involve case management related to workers' compensation and utilization of specific guidelines for care coordination.
