The role includes care coordination and care transitions planning throughout the acute care patient experience. This position collaborates with the physician, utilization manager, medical social worker, and bedside RN. The focus is on assuring timely progression and transition of patients to the appropriate level of care. The goal is to prevent unnecessary admissions or readmissions. The care management process encompasses communication and facilitates care across the continuum through effective resource coordination. The aim is to achieve optimal health, access to care, and appropriate utilization of resources. This is balanced with the patients' self-determination while coordinating in a timely and integrated fashion. Collaboration occurs with patients, families, physicians, the interdisciplinary team, nursing management, quality, ancillary services, third-party payers, and community resources. Key job accountabilities include: Patient initial and continued assessment to gather medical, psychosocial, and financial information. Determining moderate or high-risk levels for readmission. Conducting screenings for ancillary supportive services. Leading the healthcare team in developing comprehensive care coordination plans that meet clinical needs. Identifying and referring quality and risk management concerns for patient safety reporting. Utilization management requires reviewing medical records to ensure patients continue to meet level of care requirements. Collaboration with attending physicians to confirm necessary documentation is crucial. Expediting transition planning for patients who no longer require an acute level of care is essential. The role monitors length of stay and addresses outliers. Assuring timely transition planning and necessary documentation for post-acute transfers is critical. Care coordination includes formulating transition plans based on available care options. Regular assessment and communication with the healthcare team are vital for effective transition planning. Identifying barriers to care progression and resolving them with the multidisciplinary team is expected. The role demands the review and modification of care plans to ensure timely transitions. Communication with financial counselors regarding insurance issues is also necessary. Participation in ongoing departmental operations is important for identifying new processes and protocols. Contributing to cost-effective practices ensures optimal patient outcomes and resource utilization.
