The On-Call LVN works with the Health and Wellness team in collaboration with the Addiction Medicine team in the provision of treatment with Medication-Assisted Therapy (MAT) for clients with Opiate Use Disorder (OUD) and other substance use disorders (SUD). As an inpatient review nurse, you will assist patients through the continuum of care in collaboration with the patient's primary care physician, facility case manager, discharge planner and employing contracted ancillary service providers and community resources as needed.
