Description

Start Date: 08/31/2026 (ASAP)
Duration: 13 weeks
Shift: Days, 5x8 (Monday–Friday business hours; shift times between 0700–1700)

Responsibilities:

Coordinate pediatric patient care from admission to discharge.
Develop and manage discharge plans.
Assess patient needs and arrange post-discharge services (DME, home health, etc.).
Perform utilization review and verify medical necessity.
Collaborate with physicians, nurses, social workers, and therapists.
Document care in Epic EMR.
Ensure compliance with hospital and CMS guidelines.
Educate patients and families and facilitate safe, timely discharges.
Float within Case Management services as needed.
