Job TitleRegistered Nurse - Case Management (Hospital)

Schedule / Job TypeContract | Days | 5×8-Hour Shifts (8:00-16:30) | Every Other Weekend Required
Key Responsibilities- Provide case management services within a hospital setting, coordinating patient care from admission through discharge
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Float between like units based on facility need
- Document all patient interactions and care coordination activities using Epic
- Participate in holiday coverage, with an expectation to work 3 of 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
Required Qualifications- Active RN license; compact license must be in hand at time of submission
- Minimum of 2 years of case management experience; at least 1 year of relevant experience required
- Acute care experience required
- Proficiency with Epic EMR
- Must submit all requested time off (RTO) at the time of initial submittal; post-offer RTO requests may not be considered
- Local candidates are welcome (locals accepted)
Preferred Experience / Skills- Prior travel nursing experience in a case management or care coordination role
- Experience working within a large health system or multi-facility environment
- Familiarity with utilization review and discharge planning processes
